Flying Horse Foundation forms

Thank you for your interest in the Flying Horse Foundation youth programs. We are excited to meet you and your family! To get started you will need to fill out several forms. Once completed you will be able to schedule your tour at the Flying Horse Foundation facilities.

You will be required to fill out several forms in order to participate in Flying Horse Foundation Programs.

Plan for enough time to finish in one sitting.

• We do not save your progress, so if you need to stop mid-process, you may come back to this page and start over from the beginning.
